Frequently asked questions

What is an Ishikawa diagram?

It is a quality tool, created by Kaoru Ishikawa, for finding the causes of a problem. The problem (the effect) sits at the fish head and the causes, grouped into categories, sit on the bones. It is also called a fishbone or cause and effect diagram.

What are the 6Ms?

The six most used categories: Method (how the work is done), Machine (equipment), Manpower (people), Material (supplies), Mother nature or environment (place, weather, conditions) and Measurement (how things are measured and controlled). You can swap them for others that fit your problem better.

How do I write the causes?

The first line is the problem. Each category goes on a line with no indentation, and its causes come below it, each on a line starting with spaces. You can use 1 to 8 categories.
